ANTROCHOANAL POLYP

Antrochoanal nasal polyp is a prolapsed, pedunculated, painless, pearly white oedematous nasal mucosa, lining the nose and maxillary sinuses.

It is also called Killian’s polyp.

  • Antral, which is a thin stalk.
  • Choanal, which is round and globular.
  • Nasal, which is flat from side to side.
It has three parts.
  • Intramaxillary portion
  • Intranasal
  • Choanal components
Treatment:
  • a) Functional Endoscopic Sinus Surgery (FESS): It is the treatment of choice and can be done along with microdebrider. Now a days coblation is also used with microdebrider to remove the polyp from nasal cavity.
  • b) Caldwell Luc Surgery: Rarely done now a days. Performed only for recurrent antro-choanal polyposis.
  • c) Nasal Polypectomy: Polyps are avulsed using nasal snare. However, the basic pathology in the sinuses is not eliminated.

    A) Medical Treatment:
    • Not much useful in antrochoanal polyp.
    • Antibiotics, decongestants, analgesics can be used along with surgical treatment.

    B) Surgical Treatment:
    • Functional Endoscopic Sinus Surgery (FESS): It is the treatment of choice and can be done along with microdebrider. Nowadays coblation is also used with microdebrider to remove the polyp from the nasal cavity.
    • Caldwell Luc Surgery: Rarely done nowadays. Performed only for recurrent antrochoanal polyposis.
    • Nasal Polypectomy: Polyps are avulsed using nasal snare. However, the basic pathology in the sinuses is not eliminated.
